The Accident Investigation Bureau (AIB) Nigeria yesterday said fuel exhaustion at low altitude, which led to loss of control, caused the crash of a Quorum Aviation Limited’s Bell 206B3 helicopter into a residential building in Opebi, Lagos, on August 28, 2020.
The AIB also cited other factors that caused the crash in which three occupants died, as: lack of efficient management supervision of the airline’s flight operations.
The agency faulted what it called the inadequate safety oversight of Quorum Aviation Limited.
Speaking on the release of the accident report, AIB’s Chief Executive Officer Akin Olateru said: “The reports we are releasing today cover the period between 2018 and 2020. While two are classified as accidents, two are serious incidents and one safety bulletin.
“The newly released reports make a total of 72 accident reports released by the bureau since its establishment in 2007 and a total of 53 reports released since the inception of this administration.
“The five reports of today contain a total of 12 safety recommendations, which are addressed to the regulatory body, the affected airlines, the Federal Airport Authority of Nigeria (FAAN), among others.
“The bureau has now released 232 Safety Recommendations since inception with 151 of the recommendations released by this administration.”
