    Moreover, from pain to swelling or limited use of the joints, may follow other frustrations such as poor sleep, depression, anxiety, irritability, anger and aggressive behavior. The sufferer will discover that other daily or usual functions become difficult. Example may include inability to move about in the home, inability to shower, cook, have sex, play with children, inability to drive, engage in writing and attend meetings of peers. If the joints of the mouth and jaws are affected (temporomandibular joint arthritis), talking and chewing may be severely curtailed.   Some arthritis, of say the hand and hips may lead to disfigurement and hence embarrassment. Some body parts may shorten leading to visible abnormalities. Social standing in the society may begin to suffer. Further, arthritis may affect one’s profession and leading to early retirement or simply limit one’s ability to earn an income. There are many sufferers that had given up on their profession because of the crippling nature of their arthritic diseases.  How could a surgeon operate if his hands are crippled with arthritis? How can a bricklayer, driver, writer, a professional boxer, athlete and so forth operate to earn a living when their hands or legs are severely disabled by arthritis?

    Now, that we have discussed the many faces of arthritis. Arthritis, as we saw earlier, is clearly not what the popular opinion and beliefs are. It goes far and beyond the usual migratory joint pains. We saw also that arthritis can cause a lot of disabilities such as pain, joint pains, swelling, poor joint movements, body disfigurement, loss of income, loss of profession and careers.  We also saw that arthritis may lead to social and biological disenga-gements.

    Now, we will take a look at what you along with your doctors, can do to relieve the condition of arthritis.

    In our traditional way of lives, our grandparents may have applied cold or warm water and menthol to the joints which may actually have given a limited relief. There are other traditional remedies which may have helped as well. As we saw under the causes of arthritis, such superficial treatments may not have addressed such serious illnesses as blood cancers or such disease entities as sarcoidosis,  systemic lupus, sickle cell diseases and under-performing thyroid diseases.

    The first step in finding solution to the problem of arthritis is to admit you have a problem and you need solutions. While arthritis caused by malaria and typhoid may go away when those illnesses are treated, arthritis caused by osteoarthritis and overacting hormones such as hyperthyroidism or diabetes may not so readily disappear.

    Look for a registered doctor of your choice who will listen to your story in detail. It’s highly likely that the attending physician will examine you thoroughly, take blood for relevant tests which may probably include as for in our environment:  scleroderma, systemic lupus and infections such as TB. The doctor will also keep an eye on various likely cancers that may cause you joint pains. I elaborated on causes of joint pains last week.  If there is associated fever, the doctor will determine if typhoid or malaria is the cause of joint pains especially if the pain is of recent origin.   • Arthritis: Joint pains ( 1)

    Arthritis is a general term form of disease indicating inflammation of the joint(s).  In the culture of Yoruba in the South West of Nigeria, arthritis is often referred to as “awoka” meaning migrating pain.

    “Awoka” is a right description for arthritis that affects multiple joints.  This form of arthritis is common especially as one ages.  Arthritis is still arthritis even it affects only one joint.

    However, arthritis is not merely “awoka” as discussed above. The arthritis that are widely known and referred to as “awoka” frequently falls into the category of arthritis known as osteoarthritis. I am certain different cultures and languages do have their own names for migrating joint pains. Arthritis covers many disease entities as we shall see below.  Some of the origin of arthritis are deadly and may result in fatality or cause considerable disability as we shall see below.

    What causes arthritis?  There are over one hundred causes of arthritis.  However for our purpose, I will merely outline the different types of arthritis as follows. There are some arthritis whose causes are not known. This is called Polyarthritis of Unknown Origin. Into this category falls rheumatic and psoriatic arthritis. Another group is Connective Tissue Disorders which has as its members such diseases as scleroderma and systemic lupus erythematosus. These latter two diseases are very common amongst the black race.  Yet another group is Rheumatic Fever. Rheumatic Fever though not unknown, is rare in tropical Africa including Nigeria.  Low back syndromes, shoulder/hand syndromes may give rise to another group called Non-Articular Rheumatism. Sarcoidosis which is essentially a disease common among black or African descendants can give rise to a different class of arthritis.

    Infections can frequently give rise to another category of arthritis: tuberculosis, syphilis, gonorrhea, typhoid, yaws, infection with staphylococcus can all cause infective arthritis. Fungus, viral infections even parasites such as malaria as we all may have experienced at some point in our lives, could all cause arthritis no matter how short the arthritis may be.  Infections are very common in our environment and is probably responsible for most of the arthritis that we encounter on daily basis.  Of course it goes without saying that getting older is also a major contributor to arthritis in our community. For this reason, osteoarthritis due to wear and tear of our body and joints is what people commonly associate with old age and arthritis.

    Further, another group of arthritis may be due to trauma and what doctors call neurogenic (brain or nerve) disorder. Examples are arthritis due to falls, injuries, accidents (generally trauma), diabetes and joint dislocation.

    Arthritis in some people may be caused by a category called chemical imbalance or hormone disorders. Examples are: under-functioning thyroid, lack of vitamin C (scurvy), gout and sickle cell disease. Sickle cell disease is extremely common in our community and majorly a disease associated with black race or Africans

    Cancers and tumors can also cause arthritis. Cancers and tumors in this category include leukemia, multiple myeloma and cancers that had spread to elsewhere in the body.  Some innocent tumors of the joints may also cause arthritis.

    Moreover, some people suffering from allergy may also suffer from arthritis. Allergy to drugs or medicines and allergy to foods of any sort may lead to arthritis.

    Some individuals are unfortunate in having inherited some diseases that may ultimately lead to arthritis. Belonging to this category are such diseases as Marfan’s Syndrome and Ehlers Danlos Disease.

    Finally, arthritis may occur due to manifold of unrelated diseases such as over-swollen bones (Hypertrophic osteoarthropathy) and many other disease states.

    As the reader can now see, arthritis goes beyond “awoka” or migrating joint pains due to “cold water”, cold weather or old age as our people believes.  I have no doubt that warm weather and the Sun positively improves our wellbeing and improves many form of arthritis as we shall see under treatment of arthritis.

    How do you know you have arthritis and what are its consequences? Pain is a consistent manifestation of arthritis whatever the cause may be. The pain may occur on a single joint or multiple of joints at the same time. The may be limited a single joint of be migrating to different joints at different times. Arthritis may affect small joints such as fingers or moderate size joints such as neck or the big joints of the body such as hip, knee, lower back, shoulder and ankle.   The joints may or may not be swollen and warm. Crucially, the joints and the individual may experience limitations in the movement of the respective joints. Once there is a limitation in the movement, then the function or use of the joint may be severely limited. The longer the use of the joint is limited, and no action is taken to remedy this situation, there is a likelihood that the disease of arthritis will get worse over time.

    Unknown to many, food plays a vital role in preventing or managing arthritis. OYEYEMI GBENGA-MUSTAPHA writes.

    Nutrition can  help prevent or  manage arthritis, a traditional practitioner, Dr Segun Fahuwa, has said.

    In an interview, he said what one eats or does not is the first step to avoiding the disease.

    Fahuwa, popularly called Mister Guarantee, said eating processed foods, such as baked or prepackaged meals and snacks could aggravate arthritis because they contain trans-fats which preserve them. He added that trans-fats trigger systemic inflammation. To dodge trans-fats, he said, avoid any food which contains partially-hydrogenated oils.

    Sugar is another food that must be shunned. He explained that foods that contain refined sugar, such as pastries, chocolate, candy, soda, and even fruit juices—trigger the release of proteins in the body called cytokines, which cause inflammation. Sugar is labelled in  many ways in food items, he warned, adding that one should watch out for corn syrup, fructose, sucrose, or maltose.

    Meat, especially red meat and fried foods, could be dangerous to one’s health, because they are high in saturated fats and cholesterol, he said. “Meat contains high levels of advanced glycation end products (AGEs) that stimulate inflammation, particularly when it is broiled, grilled, roasted, or fried. But it’s not just fried chicken you should avoid, though. Other fried foods, such as dough nuts and French fries, contain trans-fats in addition to AGEs,’’ he said.

    Refined grains, such as white bread, white pasta, and crackers, Fahuwa warned, also cause the glucose level to rise, adding that they must be avoided.

    “Refined grains (as opposed to whole grain products) cause a spike in blood glucose level, which has been shown to increase levels of several inflammation-markers in the body. Another factor to consider is sensitivities before switching to whole grain though, as people with gluten (found in grains) sensitivities may experience joint pain and inflammation from consuming wheat products,”  Fahuwa said.

    He added: “Cheese and high-fat dairy, such as butter, cream cheese, margarine, and mayonnaise are all high in saturated fats and AGE. They’re big inflammation triggers and should be consumed sparingly.”

    Avoiding foods and drinks that trigger inflammation is not just good for your arthritis alone.  Fahuwa said an anti-inflammatory diet could also help prevent other chronic conditions, such as heart disease and diabetes.

    “Corn, peanut, sunflower, safflower, and soy oils are high in omega-6 fatty acids are healthy in small doses. But excessive omega-6 consumption can trigger inflammatory chemicals. Before you despair that everything tasty seems to be off-limits if you want to practise an anti-inflammatory diet, keep in mind that a few simple swaps can often make the difference between healthy and unhealthy food choices. For example, cook with canola oil instead of corn oil, or swap out white bread for whole grain bread. This is not the case for the types of oils rich in omega-3 fatty acids, such as olive, canola, and flaxseed oils. These varieties are healthy, even in larger amounts,” Fahuwa said.

    He said there are oils that can fight off arthritis. “All oils are a mixture of fatty acids- monounsaturate, polyunsaturated and saturated – and it’s the ratio of these acids that determine whether an oil or fat is healthful or harmful.

    “Healthy oils and fats have a higher amount of unsaturated fatty acids and a lower amount of saturated fatty acids than their less-healthy counterparts,” says Haas.

    “Unsaturated fats – mono and poly – have unique health benefits. Monounsaturated fats can help lower your blood LDL [bad cholesterol] level and raise HDL [good] cholesterol, which in turn can help prevent cardiovascular disease. Polyunsaturated fats may lower total blood cholesterol, which also helps prevent heart disease,” explained Fahuwa.

    He pointed out that at the other end of the spectrum are saturated fats, such as butter, which are solid at room temperature and are linked to unhealthy cholesterol levels and heart disease. This is a type of fat you should limit in your diet.

    ‘’Squeeze the most health benefits out of your oils by understanding their best uses, which often depend on their smoke point. This is the temperature at which different oils begin to smoke and break down, which destroys the compounds that give them their health benefits.

    “Finer oils with low smoke points are not good for most cooking applications because cooking destroys their nutritive value,” Haas said.

    She advised reserving oils with low smoke points for dishes that don’t involve high heat or for drizzling on soups and vegetables just before serving. ‘’Storing oils properly will keep their taste and beneficial compounds intact. Air, heat and light speed up deterioration, and most should be kept on a cool, dark shelf. Some oils, particularly those high in polyunsaturated fatty acids, go rancid quickly and are best stored in the refrigerator and brought to room temperature before using. If oil has an unpleasant taste or odour, it’s time for a new bottle,” Fahuwa added.

    New research studies published in the Nature Reviews Rheumatology journal provide evidence to corroborate Sear’s theory. Sears states, “In a study published just a few weeks ago in the journal Nature Reviews Rheumatology, researchers followed 6,616 people for more than 10 years. They found that those who had moderate to severe gum disease had more than twice the risk of developing rheumatoid arthritis.”

     

    Sears continues, “Other research suggests that clearing up gum disease could help prevent or treat Rheumatoid Arthritis. Case Western Reserve University School of Dental Medicine found that people who were treated for gum disease had significantly more improvement in their Rheumatoid Arthritis symptoms than those who were only treated for their Rheumatoid Arthritis.”
