Nigeria’s total fertility rate drops to 4.8 per woman in five years

The 2024 Nigeria Demographic and Health Survey (NDHS) Report said Nigeria’s Total Fertility Rate (TFR) has declined from 5.3 children per woman in 2018 to 4.8 in 2024, marking a major demographic shift over the past five years.

The Minister of State for Health and Social Welfare, Dr Iziaq Salako, disclosed this on Friday in Abuja at the launch of the report.

Salako said the decline reflects gradual gains in access to and use of family planning services nationwide.

He said;: Modern contraceptive use among currently married women increased modestly to 15 per cent in 2023 from 12 per cent in 2018, while satisfied demand for family planning rose to 37 percent”.

He noted that although the improvements were encouraging, they remained below the levels required to drive rapid social and economic progress.

According to him, antenatal coverage currently stands at 63 per cent, skilled birth attendance at 46 per cent, while postnatal coverage within two days after delivery rose from 38 per cent in 2018 to 42 per cent in 2024.

Salako said the under-five mortality rate had dropped significantly from 132 deaths per 1,000 live births in 2018 to 110 per 1,000 in 2024, while neonatal mortality remained nearly stagnant, moving from 39 to 41 per 1,000 live births within the same period.

“More efforts are required to reduce neonatal deaths, which account for about 40 to 45 per cent of under-five mortality,” he added.

The Minister said the findings highlight urgent gaps requiring coordinated responses, adding that the ministry had already begun translating them into policy reforms.

He said initiatives such as the Maternal and Maternal Mortality Reduction Initiative and the Nigerian Child Survivor Act (2023–2025) were designed to address context-specific challenges through coordinated interventions.

Salako added that the data would also guide sub-national governments to identify geographical areas requiring urgent intervention.

Chairman of the National Population Commission (NPC), Hon. Nasir Kwarra, said the NDHS remains a vital instrument for understanding population trends, child and maternal health, nutrition, malaria, HIV, and other development indicators.

Kwarra said the survey was implemented by the NPC with oversight from the Ministry of Health and Social Welfare and support from development partners.

He added that the success of the exercise demonstrated the power of institutional collaboration and reaffirmed that “data is not merely a technical output but a public good that empowers evidence-based governance”.

World Bank Senior Health Specialist, Dr Ritgak Tilly-Gyado, said the data would support the Bank’s analytical work and policy modelling in key areas such as health, education and nutrition.

  • Female formula, a 2025 hope for fertility and pregnancy

Female formula, a 2025 hope for fertility and pregnancy

Female Formula should always be at the beck and call of anyone who is treating female infertility challenges, whether primary or secondary.

This product is one of the 30 plant medicines produced and marketed in Nigeria by Edible Herbs for a wide spectrum of health challenges.

Edible herbs or female formula is a unique combination of herbs that not only balances hormones, treats infertility by stimulating ovarian functions but, also, takes care of menstrual problems.

Its formulation include but is not limited to the following ingridients, one of the stars of which is Lady’s Mantle.

This plant rings a bell whenever problems in the female reproductive system are to be addressed.

Herbalists believe its shape in lobes and notches is a signature tune. By this, they mean the shape is a secret code for which organs in the body a herbal plant is created to maintain in good health and to cure when it is healed. Lady’s Mantle resembles the entire reproductive system of a woman, especially the uterus.

What benefits can a woman’s reproductive system derive from her regular consumption of Lady’s mantle in diverse ways? 1.

Lady’s Mantle strengthens muscles of the uterus.

The muscles need to be strong for several reasons. If they are weak, Prolapse of The Uterus may occur. This means the entire uterine structure may collapse like a pack of cards or like the decking of a house constructed with inferior building materials.

When this happens, parts of the uterus may extrude into the cervix and, from there, sometimes into the vagina.

It can be embarrassing for a woman, to say the least,if she has to always use her fingers to push the extruded prolapsed uterus back into her body, especially during sexual intercourse.

Weak uterine muscles may not support a baby growing in the uterus, because this amounts to additional load and pressure on them.

Where the muscles of the cervix are weak, this may cause incompetent  cervix in which weak muscles of the cervix give way about mid-term in what becomes spontaneous or habitual abortion for some women.

When this happens, the doctor may have to surgically hold muscles of the cervix together in what is called cervical cerclage or surgical suture.

The cervix is the protective outer corridor of the uterus which connects it with the vagina.

Older women, too, need to strengthen their uterine muscles to prevent prolapse of the uterus.

They are prone to this, especially in menopausal years, when hormonal imbalances deprive them of enough estrogen, the star hormone of women to maintain their muscles, nerves and bone in youthful forms.

For Lady’s Mantle also helps to balance the hormones. 2. Lady’s Mantle prevents and stops bleeding in the uterus and detoxifies it of those toxins which can cause systemic problems. Bleeding can make the uterus to become suboptimal.

Deprive of oxygen and nutrients, the cells become weak and are unable to function optimally. In some cases, it causes an establish placenta, through which the foetus feeds, to rupture and to bleed, and this may be dangerous for the baby within.

Through its tanin component, lady’s mantle may help to stop the bleeding. Lady’s mantle may be a super star gynaecology herb, but it is by no means a lone tree in a forest. Edible Herbs recognises this in the formulation of Female Formula which, therefore, comes with three other starry herbs for reproductive system health.

These other reproductive system starry herbs which women of all ages should not ignore are a. Fertility Tree, b. Red Clover and c. Wild Yam.

I wish to play a prank with Fertility Tree because many women, especially readers of this column, have often had of it or probably take it as a supplement with good reasons, but can hardly recognise it by this name.

The name fertility tree comes from the Greeks and the Romans. In Greek mythology, the tree is believed to be associated with Aphrodite, the goddess of love and fertility.

The Romans, on the other hand, associated it with Venus, the goddess of love and beauty. How would a regular user of Chaste Berry feel when I say that Fertility tree and Chaste Berry are the same plant medicine! That is Prank number one that some naturopaths may play. This herb does more than make a woman fertile and pregnancy prone. I am aware of a woman who would be reading this and recalling her days of fertility and pregnancy troubles which this herb, along with lady’s mantle and squaw veine, reversed with beautiful results.

Her first pregnancy came with serious spotting which warranted complete bed rest for about two weeks.

The next two or three pregnancies came down mid-term because of hormonal imbalances.

By the time she tried again to get pregnant, I had come by the knowledge of these herbs during a meeting in the United States where an American pharmacologist reported that they stabilised his wife’s reproductive system after about three miscarriages in a row.

I shared this experience with many women on my return home and received jubilant gratitude from them. What many women do not know and which I seldom tell them is that this Chaste Berry can be a double edged sword.

For while it may balance the hormone estrogen against the hormone progesterone to stabilise their productive system, it may help to put a flaming Romeo in chains.

That was why, in the Middle ages, Fertility Tree was also called chaste Berry by Europeans.

The word Chaste is associated with Chastity, the human limit in the pursuit of Purity, which derives from Pure or Purity.

Only the Almighty Creartor is Pure or Holy! As man cannot, therefore, be pure, the limit set for his specie in the striving for Purity is Chastity or Faithfulness. In the European monasteries of old, Chaste Berry was given to the monks as part of their diet to Clip or to Droop the wings of their Libido.

What I am saying in effect is that, in men, chaste Berry can inactivate the habit of men running around with almost everything in skirt.

Generous portions of red meat in the diet is believed to aggravate the sexual instinct of men. Chaste berry is believed to quieten the sexual instinct. I seldom mention this aspect on chaste berry because it may be counterproductive to a woman who steals it into the diet of her man just to keep him, bind him, hand and foot, to herself alone. What if he came to know what went wrong and the source? Has the market no more supplies of Long jack, refire, libido tonight, steel libido, robust roots, African Black Ant, Dman and the likes of them to get a cold fish man back into fiery form? If you still cannot guess the landing port we are heading for, here it is…. Vitex is what we have been talking about and calling Fertility Tree or Chaste berry!

Almost every woman who has faced hormonal challenges knows about it.

I  have heard that some naturopaths get it to some men who have testosterone problems and prostate gland challenge,  but I cannot vouch for its use and effects in this scenario.

In 1990s, I often suggested that women combine Vitex with Phytoestrogen and Cardiotonic Pills from Tasly. The effects were often wondrous when they addf Milk  Thistle and Zinc. Phyto-Estrogen was a proprietary blend of plant estrogens. In the bodies of some women, their own human estrogen can be too harsh for their reproductive system.

This problem can be worse when estrogen overpowers progesterone or the woman is estrogenated.

She may suffer from mild to serious  pre-menstrual syndrome (PMS),bloating, swollen or sore breasts, food cravings, mood swings, irregular, scanty, excessive, painful menstruation or absence of it. Uterine muscles may ache, become inflamed, suffused with arachidonic acid, and uterine fibroids may develop. Some women suffer from single or polycystic ovarian syndrome in one ovary or both. To add to such discomforts, some women are driven to vomit and experience peace only after they have vomitted a greenish, bitter substance. Some of them ask me what all of these mean.

This subject offers an opportunity for an explanation. The greenish, bitter substance is likely to be bile, a digestive salt produced by the liver and stored in the gall bladder for the digestion of fats and other purposes. When estrogen and other hormones run riot during menstruation, hormone-like substances called prostaglandins produced in the uterus stimulate cramping or spasms of soft muscles in the body, upsetting the digestive system, causing nausea and voitting. In Traditional Chinese Medicine(TCM), this is seen as a Rebellion of the Life Force, which is meant to eliminate unwanted forms downwards but instead prefers an upward spiral.

This is often associated with a Stagnant Liver. Thus, while anti-spasmodic herbs such as Blue Vervain and Basil  Ocinum may be used to quieten the spasms, and the liver may profit from Milk Thistle protection, detoxification of the liver is imperative.

Estrogen Sister That is what I call Estriol, Estrione and Estradiol. Scientifically, they are Estrogen fractions or splittings.

They are the three major “sisters” or components of human estrogen. Estriol is the weakest followed in terms of harshness or potency by Estrione and Estradiol. Estrione is often used in Hormone Replacement Therapy ( HRT) for post-menopausal women with all the possibilities of a cancer aftermath. estradiol finds use in some contraceptives. Women react differently to them. What may hurt one woman may not injure another.

Plant medicine care givers recommend plant estrogens or phytoestrogen when a woman has problems with her own body’s estrogen.

This is because plant estrogens are weaker and tolerated more by the body.

For example, human estrogens may be 100 to 1000 times harsher than plant estrogens. Isoflavones, a common plant estrogen from Soyabean, is about 10 to 20 times less harsh or potent than human Estrogen.

To enjoy plant estrogen, a number of steps should be taken (a)The body, especially the liver and the kidneys, should be detoxified .

(b)Bitter things should be eaten to stimulate the liver, the body chief detoxifying organ c.

    (c) Stuff like milk and poultry egg should be eliminated in the diet.

Milk comes with estrogens in cow feed to stimulate their breasts to make milk almost every day.

Egg is laden with estrogen in poultry feed to stimulate hens to lay egg almost everyday.

The estrogen residues increased the estrogen load in a woman’s body and does the same for a man. d.

    (d) Food should not be wrapped with cellophane, as is the current practice in Nigeria with the rapping of eba, fufu and amala, among others.

The cellophane is a soft plastic which leaches xeno estrogens, petrol residue-derived estrogens, into the food.

Sun-heated”pure water” sachechets and sun-heated bottled water also introduce xeno estrogens into their contents. (e) When the liver breaks down excess human estrogens, the diet should be fiber-rich to move the fragments out of the body through the stool , otherwise they may be re-absorbed in the colon to worsen the estrogen loads.

Consumption of phyto estrogen should be a daily feature of the diet of young and older women. In pre and post menopause, the ovaries reduced their estrogen production.

Estrogen makes a woman’s body a woman’s body. With estrogen depletion in pre-menopausal and post-menopausal years, the female body begins to give way in all sorts of ways. The hair easily breaks and falls, vision dims, hearing loss may occur, teeth and gums exhibit problems, the breasts are a shadow of their old selves, the great twinggy and curvy structure disappears and the bones become porous, weak and may easily break, sometimes making hip replacement a necessity. This is why it pays off to give the body phytoestrogen support.

Resort to chemical HRT sometimes ends disastrously. To Lady’s mantle and Vitex, therefore, may be added the proprietary formula Phytoestrogen from Tasly and Zinc, the fertility mineral. Red Clover Here comes the third of four components of Female Formula.

It is many sided in its health-promoting activities. It is supportive of the immune system.

Its chemical substances are reported to lower blood cholesterol levels, thereby reducing blood pressure and improving cardio vascular health. Bone density may receive support from Red clover. Thus, osteoporosis may be kept at bay and the risk of fractures eliminated.

The herb is also anti- inflammatory and bears anti-oxidation potentials. It synergies with Lady’Mantle and Vitex in uterine health protection and advancement.

It may help to bar the way for uterine fibroids, endometriosis and other uterine related questions. In terms of fertility, Red clover may be counted upon in the improvement of egg quality and the regulation of ovulation and other reproductive processes.

For menopausal women, a good news is that Isoflavones of Red clover have been noted to help reduce and stamp out night sweets, Hot flashes and mood swings. Wild Yam. You must wonder why This yam specie is called Wild Yam. It belongs to the botanical family of the Edible yam tuber you can boil, roast, fry or pound, but it is inedible because it is Toxic when it is eaten raw. Nevertheless,it has been used as a plant medicine in North America for hundred of years because of its healing properties. It’s active medicinal ingridient is Diogenin.

It was thought that Diogenin could be converted to progesterone

That was why it’s featured in creams used as in HRT or to ease menstrual cramps.

However, it is now well confirmed that, while a conversion is possible in the laboratory, the body is unable to transform diogenine into progesterone to balance out Estrogen.

Nevertheless, Wild Yam continues to feature in female reproductive health therapies because it is found to moderate Estrogen activities and to quell inflamation, mood swings and other symptoms associated with derailment of the system.

In the 1990s, it was popular for breast development and featured especially in a product called Great Curves sold in Nigeria by nature’s way, second largest nutraceutical company in the United States after GNC.

The conclusion of modern research of this herb, therefore, is that there may be other components of Wild Yam that are doing the job but are yet unknown to researchers or that the exact modality of the way the herb works is still elusive. The North America Indians who have used it for hundreds of years, under the direction of Nature beings, cannot be foolish but wise in their intuitive sensing and cognition.

Cardio Tonic: Pill I would like to conclude that cardiotonic pills be included in female reproductive health therapies. It is now reformulated and called ICP Danshen Pill to pack in more healing force from Danshen, a popular and powerful Asian herb. In the early 2000s ,it was the rave in Nigeria for blood circulation problems, especially blood clots and strokes, Nowadays, and as at then, many women experience heavy blood clots in their menstruation, a sign of sluggish blood movement and detention or retention and backup menstruation in the uterus.

This stagnation is a possible progenitor of uterine fibroids and, perhaps, endometriosis. At that time, Cayenne Pepper, Apple Cider Vinegar and their likes, including Ferumphos in the stable of Biochemic or tissue or cell salts, helped to break the de-oxyenated menstrual clots and promote rich circulation. Cardiotonic Pills was a wonder on its own.

In an experiment, the hand of a volunteer was frozen in a freezer. It looked lifeless, seemingly with no blood in it.

Then, the volunteer was given Cardiotonic pills by mouth. In under five minutes, the frozen, immobile hand came alive with restoration of blood circulation. What was the magic?

There was one percent Camphor in the formulation. It uprooted the frozen blood, warmed it and reversed the dying hand.

That was to prove that blood clots which could cause heart attacks and stroke could be taken out.

It offers hope for uterine fibroid resolution as well, in my view. A popular current therapy now is Camphorated Castor Oil massage on the external skin of a fibriod challenged woman re-forced with a heating pad or hot water bottle in the absence of the former.

This conversation will continue another day. Meanwhile, it is my pleasure, Ladies and gentlemen, to welcome Female  Formula from Edible Herbs to the community of female reproductive health seekers to which by right also belong menopausal and post menopausal women.

  • “Future-Proofing Your Family: The Essential Guide to Fertility Preservation”

“Future-Proofing Your Family: The Essential Guide to Fertility Preservation”

    • BY OLANREWAJU ELIZABETH JUMAI

In a rapidly evolving world, planning for the future has never been more critical, especially when it comes to family and fertility. As societal norms and medical advancements shift, many are finding themselves confronted with the important question: How can we safeguard our ability to have children when the time is right?

    UNDERSTANDING FERTILITY PRESERVATION

Fertility preservation is the process of preserving eggs, sperm, or reproductive tissue so that a person can utilizeit for future reproductive purposes (Loren et al., 2013). Fertility preservation involves medical techniques that help individuals as well as couples maintain their ability to conceive in the future despite current health conditions, age, or lifestyle choices. The process has become increasingly relevant as more people choose to delay starting a family or face medical treatments that could impact their reproductive health.

    WHY CONSIDER FERTILITY PRESERVATION

*Age-Related Concerns: Women’s fertility naturally declines with age, making early preservation a viable option for those who wish to delay parenthood until later in life. For men, sperm quality can deteriorate over time, although generally at a slower rate.

    * Medical Treatments: Certain medical conditions and treatments, such as chemotherapy for Cancer, canadversely affect fertility. Fertility preservation offers a chance to have biological children in the future, even after such treatments.

    * Lifestyle and Career Choices: Busy career paths, travel, and personal choices might lead individuals to delay starting a family. Fertility preservation provides a safety net for those who want to keep their options open.

    Methods of Fertility Preservation

    * Egg and Sperm Freezing: This involves collecting and freezing eggs or sperm to use later. It’s acommon method for both men and women looking to preserve their reproductive potential.

    * Embryo Freezing: For couples who are ready to conceive but need to delay pregnancy, creating and freezing embryos can be an effective strategy.

     * Ovarian Tissue Freezing: This method involves removing and freezing ovarian tissue, which can be re-implanted later to restore fertility.

    * Testicular Tissue Freezing: Similar to ovarian tissue freezing, this method is used for preserving testicular tissue in males, especially useful for prepubescent boys or men who wish to safeguard their fertility before undergoing medical treatments.

    The Process and Considerations

The process of fertility preservation typically involves several steps, including consultations with a fertilityspecialist, who will outline the best options based on individual health and circumstance, medical evaluation andtheactual preservation procedures. Costs can vary significantly based on the techniques used and the specific clinic. Insurance coverage for fertility may not always be available, so it’s important to understand the financial implications and explore possible options for assistance.

    Long-Term Considerations

While fertility preservation offers hope and flexibility, it is not a guarantee of future success. Success rates candepend on various factors, including age, health conditions, and the methods used. Regular follow-ups withafertility specialist are essential to monitor and manage preserved reproductive materials effectively.

    Looking Ahead

As reproductive technology continues to advance, fertility preservation is becoming increasingly accessible and effective.

For many, it represents not just a medical procedure but a critical investment in their future family life. Understanding and exploring these options can help ensure that individuals have the opportunity to build the family they envision when the time is right for them. For individuals in learning more about fertility preservation, consulting with a fertility specialty and exploring available resources is a crucial first step. In doing so, you can make informed decisions and take proactive steps to future-proof your family.

Joe Khalifeh: Fertility patients face many challenges

Joe Khalifeh is a fertility specialist at the Onewellness Center. In this encounter with Yetunde Oladeinde, he talks about his experience in Nigeria, clinical women’s health, surrogate motherhood, diet, how to live healthier and happier lives as well as the infertility challenges that have become increasingly prevalent among women.

Tell us about your experience working on Women’s health and fertility issues.

Being a gynecologist and obstetrician can be both challenging and rewarding. It is so satisfying to monitor the development of the baby inside the womb and ensure the health of both mother and baby during pregnancy and childbirth. Furthermore, the field of infertility makes the job interesting and strengthens the relationship between me and my patients.

What inspired you to go into the medical field?

When I was a student, like all my colleagues, I was inspired by a deep-seated desire to make a positive difference in the lives of others. I am passionate about experiencing the joy of bringing new life into the world and giving hope to those who have lost it in their journey to get pregnant. Moreover, the intellectual challenges of the medical field and the constant advancements in medical technology and research also attract aspiring medical professionals.

What are some of the challenges in the sector?

The accessibility of women’s health treatment should be available to all women. Fertility patients, for example, face numerous challenges, including financial and emotional ones. Dealing with insurance companies and navigating the complexities of medical service reimbursement can be frustrating for them. Furthermore, there is a lack of knowledge about the importance of reproductive health for the well-being of women. This topic remains taboo for some women seeking the help of OBGYN, and they may encounter complex ethical dilemmas, such as making decisions related to contraception, fertility treatment, and cosmetic Gynecology.

What is unique about your organisation the One Wellness Centre? Our clinic represents innovation in the field of reproductive health, utilizing state-of-the-art equipment coupled with our extensive expertise from various countries. Our highly trained and professional team, including Nigerians and expats, is committed to bringing the latest technologies in the field to Nigeria. Additionally, with our Feto-maternal expertise, we ensure that pregnant women experience a smooth pregnancy with thorough follow-ups, offering all necessary prenatal tests and using the newest ultrasound machine.

What are some of the memorable experiences and achievements?

In our field, each case is an achievement and holds a special memory for me. Over the years, I have had the privilege of assisting countless women on their beautiful journey of pregnancy and delivery. One of my most significant career achievements is the successful management of high-risk pregnancies. I continuously update my knowledge and skills to ensure the well-being of both mother and baby in challenging situations, while also utilizing cutting-edge technologies to provide hope for those who may have lost it during their parenthood journey.

AI is making waves all over the world.  What changes should we look forward to?

AI-assisted robotic surgery has recently emerged in the medical field. Additionally, AI can assist in the remote monitoring of pregnant patients, allowing us to track vital signs and maternal health in real-time, as well as handle administrative tasks and record-keeping, which enables doctors to focus more on patient care. It is important to note that while AI promises significant advancements in the medical field, healthcare professionals must continue to monitor and evaluate these technologies to ensure they enhance patient care, improve outcomes, and uphold ethical and safety standards.

Fibroid is common in African Women. What has been your experience on the job?

It’s important to know that uterine fibroids are a common medical condition, not limited to African women. However, during my experience, I’ve observed that Nigerians often opt for myomectomies as a treatment regardless of the type of fibroids they have. It’s essential for all women to consult a specialist and become informed about all available treatment options. In some cases, myomectomy may not be necessary, and this can affect the chances of conceiving later on. Therefore, it’s important to discuss your case with an expert to determine the best treatment option.

Can women who have experienced early menopause get pregnant?

Women who have experienced early menopause may have a reduced ability to conceive naturally, but it is not impossible for them to get pregnant. They should consult a fertility specialist who can assess the situation and provide guidance on the most appropriate fertility treatment available, such as Intra Cytoplasmic Sperm Injection (ICSI), which involves injecting sperm directly into the cytoplasm of the eggs. These patients are also candidates for Ovarian Rejuvenation, which is a trend in our practice at One Wellness Clinic. It is an innovation in our field and has promised results worldwide and in our practice. This procedure can impact not only egg quantity but also quality.

What are some of the advantages and challenges of surrogate motherhood?

Let’s begin with the advantages: Surrogacy allows individuals who cannot carry a pregnancy to have a biological child and experience parenthood. On the other hand, surrogacy is associated with stigma and misunderstandings in our society, often leading to judgment against those involved in the process. So, it can be emotionally challenging for the couple at times.

What advice do you have for young people who would like to donate or freeze their eggs?

The most important thing is to schedule a consultation with a fertility specialist. They will provide personalized information based on your history and help you understand the process of egg donation or egg freezing. Another piece of advice is to maintain a healthy lifestyle, which can improve your fertility and overall well-being. Lastly, if you’re considering egg freezing, it’s advisable to do it at a younger age when your eggs are likely to be of higher quality.

Are there things Women should avoid eating for proper reproductive health?

Maintaining a healthy diet is essential for both men and women. While there is no specific list of foods that should be entirely avoided for reproductive health, some dietary considerations and recommendations can be beneficial, such as maintaining a healthy BMI, limiting sugar and fatty food intake, and staying hydrated. Moreover, a healthy lifestyle can play a significant role in supporting reproductive health, including regular exercise, stress management, reducing alcohol intake, quitting smoking, and getting sufficient sleep.

UK’s first womb transplant raises fertility hope for women

Medical professionals are heralding a “new era” for those wanting to become pregnant following the United Kingdom’s first successful womb transplant.

Six months ago, a 40-year-old mother of two donated her uterus to her 34-year-old sister, who had been born without one, according to a report yesterday.

Now, the recipient of the uterus is having periods and making plans for a family of her own via in vitro fertilisation.

Professor Richard Smith called the process a “massive success,” the report noted.

“We were all in tears,” he said, recalling meeting with the sisters following the operation. “It was a very, very emotional … I think it was probably the most stressful week of our surgical careers, but also unbelievably positive.”

“The donor and recipient are just over the moon.”

Conducted in February at Oxford’s Churchill Hospital, a staff of over 30 people worked over eight hours to extract the donor’s womb, and a further nine hours and 20 minutes was required to complete the operation, the report noted.

Womb Transplant UK covered the £25,000 or $31,693.50 cost of the procedure, and the surgeons and medical staff appeared to volunteer their time.

“I’m just really happy that we’ve got a donor, who is completely back to normal after her big op, and the recipient is … doing really well on her immunosuppressive therapy and looking forward to hopefully having a baby,” Smith said.

The recipient will have her sister’s womb for up to five years before it will be taken out, according to the report.

It’s “a remarkable achievement,” Chairman of the British Fertility Society Raj Mathur said.

“I think it’s the dawn of a new age, a new era in treating these patients,” Mathur said. “You have got to remember some of these patients are the most difficult fertility situations that you can imagine … they are either born without a uterus or they have lost the uterus for reasons of cancer or other problems, for instance in (labour).”

“Up until now, we have really not had any way of helping them other than surrogacy.”

The United Kingdom’s first successful womb transplant comes in the wake of the American Medical Association floating a trial balloon for uterus transplants for transgender women.

  • How ‘D &C’ may affect your fertility

The word “D & C” has been used for as long a anyone could remember. Indeed “D & C” usage in the public doman is often associated with the darker side of Gynaecology or in general, the word is frequently related to a more sinister side of medical practice.

In actual fact, “D & C” is a procedure which simply means “dilatation and curettage” which implies that the attending medical doctor makes the passage in the neck of the womb wider by dilating it with the help of a dilator and then curette by attempting to remove through gentle traction, the contents of the womb.  This procedure which I have described, is a common procedure in the field of gynaecology to address several disorders of women and specifically to take samples of the womb/uterine lining such as in cases irregular bleeding, heavy bleeding, when a growth in the womb such as polyps is to be removed, when cancer is suspected to be growing in the womb. Also, during hysteroscopy which is a procedure to pass a camera with a cable into the womb for examination of the inside of the womb. D & C is also used when an operation such as removal of fibroid is to be done.

However, D & C is often associated with the more legitimate use of the procedure in termination of pregnancy that the wider public has come to associate the word and procedure.  Millions of pregnancies are terminated in the world annually and Nigeria accounts for a reasonable share of these terminations.

To a specialist physician such as a gynaecologist, a D &C procedure may not be his first option or any of his or her options for such treatment as termination of pregnancy. On the other hand, D & C may be his only option considering the circumstances.

Now, the question remains and of course the object of this article being, why is “D & C” such a dangerous procedure in a non-specialist hand? Why is the procedure such a danger to fertility of women?

The answers to these questions lay in the skills of the operator and the reason for the operation.

For our purpose, D & C is a useful tool in the care of women: preventing undue suffering, abating heavy bleeding, preventing infection, shortening the cost of medical care and indeed preventing death of women and avoiding family grief.

When D & C is carried out by poorly skilled individuals or the procedure is inappropriately applied, a lot of damage may be done to the womb and the health of the women undergoing D & C.

For a start, the blunt instrument used in holding the neck (cervix) of the womb, may be applied so inappropriately as to tear the significant part of the neck of the womb. Further, even if this tear is avoided, the unguarded use of a dilator may cause a perforation to the womb, leading to bleeding and damage to internal organs of the woman undergoing operation.  All of these may cause fertility problems for the woman. For one, undue and too frequent dilatation of the neck of the womb may cause Cervical Incompetence. Even if the woman is able to get pregnant, the woman may suffer frequent miscarriages at about 3 months into the pregnancy.

Most significantly, most D & C that are done by inexpert hands results in heavy use of force to remove the contents of the womb in abortion procedures.  It’s this use of heavy force that ultimately removed the lining of the womb (endometrium). Once the endometrium is removed, the walls of the womb may come together and become inseparable. Once this situation of the walls of the womb become matted, a dangerous and difficult condition that is called Asherman’s Syndrome develops which is indeed a blockage in the reproductive tract of the individual.  Even if there is menstrual blood, the blood may not be able to flow out, leading to collection of blood in the upper segment of the womb and the rest of the abdominal cavity.

With Asherman’s, the woman may not be able to have regular menstruation. Further, during sex, sperms may not be able to travel upstream to meet the released eggs of the woman. Once the sperms cannot meet the eggs, fertility may not happen and the woman or the couple who may have had no fertility issues become infertile.

D & C in poorly skilled hands may also lead to infection. Infection may lead to pelvic inflammatory disease or PID. PID is not compatible with pregnancy and infertility may result.

In sum, while D & C is a vital procedure in good hands, that saves the lives of women, poorly performed D & C procedure is a danger to fertility and the lives of women. My advice would be that women should seek competent doctors and in particular to make use of the services of gynaecologist when the need arises.

  • Why your age affects your fertility

We have heard and I am certain you may have been told that, as far as life is concerned, time is everything. To extend this fact a bit to a religious level, we can take a look at a well written ageless advice:

    Let us look at the Book of Proverbs: “To everything there is a season, and a time to every purpose under the heaven: A time to be born, and a time to die; a time to plant, and a time to pluck up that which is planted; A time to kill, and a time to heal; a time to break down, and a time to build up;  A time to weep, and a time to laugh; a time to mourn, and a time to dance; A time to cast away stones, and a time to gather stones together; a time to embrace, and a time to refrain from embracing;

A time to get, and a time to lose; a time to keep, and a time to cast away; A time to rend, and a time to sew; a time to keep silence, and a time to speak; A time to love, and a time to hate; a time of war, and a time of peace. What profit hath he that worketh in that wherein he laboureth? I have seen the travail, which God hath given to the sons of men to be exercised in it.

He hath made everything beautiful in his time: also he hath set the world in their heart, so that no man can find out the work that God maketh from the beginning to the end. I know that there is no good in them, but for a man to rejoice, and to do good in his life. And also that every man should eat and drink, and enjoy the good of all his labour, it is the gift of God. I know that, whatsoever God doeth, it shall be forever.”

In no other areas of human endeavor is time so precious and meant to be used appropriately as in human reproduction especially in the female.

Let us also look at some timeless wisdom that we may borrow a leaf from to guide us in our quest to understand human reproduction and fertility.  There is a saying by the sage that as far as talents and resources are concerned especially human endowment, “if you don’t use it you will lose it” wisdom remains true. Combined with the idea of time, it means that if we don’t use our resources at the time allocated for it, age will catch up with us, the talents, resources and endowment that we have been given  will decay and we will lose our chance and ability to make use of what we have been given.

This is even truer is fertility and especially in female fertility.  Aside, if you also misuse these resources in human body and also misuse the time, infection such as pelvic inflammatory diseases (PID) and sexually transmitted diseases (STD) may ensue and damage the human reproductive parts and fertility.  Delay may also allow fibroid to grow in the womb. Note: non-breast feeding or never having had a pregnancy is a risk to having breast cancer.

Having said the above, let us now return to science.

    For Female

At the time of when babies are still in their mother’s womb and for a female, at about 14-22 weeks in the womb, a typical female baby (fetus while in the womb), carries about 3million eggs (follicles) in each of her ovaries (one on each side of the womb). At the time of birth, these eggs had fallen to about one million in each ovaries. By the time of first menstrual cycle or puberty, say about 13-15years of age), these eggs had fallen to 250, 000 in each ovaries. Fertility doctors call these follicles ovarian reserves. Of all of these follicles, only a total of 500 will be released for fertilisation throughout the life of the woman. The rest of the follicles or eggs will remain dormant or go into natural waste.

Thus, a typical female’s peak reproductive time and fertility is between ages of 22-32. Before age of 22, the physical structures supporting pregnancy is relatively immature. And besides, the individual may not be mentally ready to be a mother.

From age of 35, the ovarian reserves, begin to die off and diminish at an alarming rate so much that the fertility and reproductive ability falls off rapidly. That is not to say that women may not be naturally fertile beyond this time. Some do even into age of mid-forties. Those lucky ones are exceptions. Some may claim to be fertile up to age of 50, an age when the ovarian reserve is just about 1000 follicles that is left.  Such ability to naturally conceive at 50 is rare and women should not take things for granted or be misled especially as relates to timing of fertility.  Women, who have children or likely to have children at age of 35 and above risks having genetically deformed children such a Down’s syndrome child.  This is not to claim that all women having children beyond this age will have abnormal child. There are other risk factors at play. But again, women at the extremes of reproduction time should not take things for granted. Time is everything.

    For Male

While one is tempted to think that women are at a fertility disadvantage compared to men, or put in another way, that men are at a reproductive advantage, such ideas are really not true. Just as women undergo menopause, men also undergo fertility change. In fact, the reproductive ability of men diminish with age and from age of 65, men are likely to produce sperms that are abnormal that may give rise to mentally abnormal children.  In addition, except in some cases, just as in women, the sexual ability or sexual staminal of men diminish with age.

In summary, as we can see, time is everything. Fertility in women is more timely controlled and women desiring children should take advantage of the fertility window as I have discussed in this article.

  • Pelvic Inflammatory Disease (PID) and Effects on Fertility

PID is fairly a common illness, especially in women, who are under 50, and who are within the reproductive age group. This is not to say that PID does not occur in those who are over 50. It does, but not as frequent. The reason for this disparity is seen in how PID develops as explained below.

As the name implies, pelvic inflammatory disease is inflammation of the female pelvic area. The pelvic area is the space in both male and female that houses the reproductive organs of the respective genders.

Why are women vulnerable to PID? By the very nature of the way   the female reproductive organs are made and arranged within the pelvis, they make the female more vulnerable to infections. Also, the way the female is made to perform its reproductive functions by being the recipient party in the reproductive process, makes the deposit of sperms, germs and foreign body possible in the female pelvic cavity compared to the male gender.

    Impact of PID on Fertility

PID may cause difficulty with sex. It may cause blockade of the fallopian tubes, thus disallowing the sperms to meet the female egg leading to childlessness.

    What causes PID?

In general, PID is caused by infections. Most commonly, by infections that are sexually transmitted (STD),  but not exclusively. Among the common causes are bacteria like chlamydia and gonorrheoa-causing bacteria.

Other bacterias that are really not classed as sexually transmitted are E.coli, staphylococcus and klebsiella. Thus, PID may be caused just by any bacteria such as tuberculous bacteria and one causing syphilis that finds itself to the reproductive tract of the female.  Also, less commonly, viruses may also cause PID.  Rarely, PID may be due to chemicals agents such as chemicals of whatever origin inserted by the individual female or during surgical procedure by the doctors.

Conditions that make the female vulnerable to PID (How they could get PID?)

A woman is at risk of getting PID if she has multiple sexual partners or even a single partner with infection. In such cases, risk of transfer of infection from one partner to the other is high. Childbirth, getting, contraceptive device called IUCD, during surgery such as endometrial sampling of the womb lining, during abortion or miscarriage predispose women to PID.  Having sex before  20 years carries the risk of PID for the woman. The infection from the vaginal migrates towards the cervix and tubes to other parts of the pelvis to spread the infection.

What are the Symptoms of PID?

You may not even have a symptom at all and you may never know you have PID until very late when complications had set in. Fever may occur in acute case of PID depending on the cause. Yet, in symptomatic persons, fever, pain in the abdomen, swelling of the lower abdomen, vomiting are what sufferers could experience along with vaginal discharge that may be offensive.

When the doctor sees the patient, the sign could be one of severe tenderness in the abdomen along with swelling and vaginal discharge.

In chronic PID, pain may endure and swelling and vaginal discharge. However, complications may have set in with collection of pus in pockets within the pelvis (pelvic abscess). Vaginal discharge may continue.

    Complications

The entire or parts of the reproductive tract may be damaged beyond repair, especially the tubes may become blocked and cause infertility. Other organs may become matted and inseparable thus causing continuing pain, swelling and discomfort. In both acute and chronic PID, sexual activity may become painful and difficult leading more to fertility and relationship problems.  Even if pregnancy occurs, a woman suffering from PID risks having ectopic pregnancy.  PID may also cause oophoritis or inflammation of the ovary as well as salpingo-oophoritis.

    Treatment

Any pain located in the pelvis in a woman should be taken seriously and a visit to the doctor to determine if it’s PID or other causes of pain, is important.  Almost certainly, some tests and physical examination of the victim will be carried out and appropriate treatment including antibiotics and pain killer given. The sexual partner(s) will also need to be treated to avoid re-transfer of infection.

    Prevention

Stick with one sexual partner who has no infection. Use condom if having additional sexual intercourse with a different sexual partner. The absolute prevention is abstinence from sex. Get your partner tested for STD before you start to have sex. You may also get screened yearly for STDs.

Get to your doctor as soon as you have any of the symptoms that are discussed here.

  • ‘Taking painkillers during pregnancy may harm baby’s future fertility’

Pregnant women who take painkillers could be harming the fertility of their unborn child, and even that of future generations, a new study has showed.

Researchers from Edinburgh University looked at the effects of paracetamol and ibuprofen on samples of human foetal testes and ovaries.

The results indicated that ovaries exposed to paracetamol for one week had more than 40 per cent fewer egg-producing cells. The effect of ibuprofen was even greater as the number of cells was almost halved.

Girls produce all of their eggs in the womb, so if they are born with a reduced number it could lead to an early menopause, experts say.

The studt showed that painkiller exposure during pregnancy could have effects on unborn boys as well.

Testicular tissue exposed to the drugs had around a quarter fewer sperm-producing cells.

Scientists found that the drugs can trigger mechanisms in the cells that make changes in the structure of DNA, leaving “marks” that can be inherited.

As a result, the effects of painkillers on fertility may be passed on to future generations.

“We could encourage women to think carefully before taking painkillers during pregnancy and to follow the existing guidelines — taking the lowest possible dose for the shortest time possible,” said Dr Rod Mitchell, who led the research.
