    NSIB begins probe of derailed Abuja-Kaduna train

    The Nigerian Safety Investigation Bureau (NSIB) has commenced investigation following the derailment of Abuja-Kaduna train on Sunday.

    The train derailed at Jere Station departed Rigasa at 8:00am with 685 passengers and crew onboard. 

    The passenger train KA2/loco 2702  was en route to Idu Station, Abuja from Rigasa Station, Kaduna.

    The Director of Public Affairs and Consumer Protection of the Bureau, Mrs. Bimbo Oladeji in a statement in Abuja on Monday stated that the baggage van (BVA) and one passenger coach derailed. 

    She noted that the derailed coaches were dragged approximately 86 feet from the switch point before coming to a stop.

    She said as part of the investigation process, the Bureau would analyse data from the On-Train Monitoring Recorder (OTMR) and other critical sources of information.

    She added that the investigation to be carried out would examine the train’s speed, track conditions, mechanical components, and operational procedures.

    Giving details on the number of those on board, she said: “685 passengers and crew were onboard. Crew members onboard included two drivers (one at the front and one at the rear locomotive), two train guards, three Traveling Ticket Collectors (TTC), 20 police escorts (2 at each coach) and 4 bomb disposal unit members.”

    The train, according to her, consisted of 10 coaches, one baggage van (BVA), and two locomotives (one at the front and one at the rear).

    On the state of the passengers and affected facilities, she said: “Thankfully, no passengers or crew members were injured. Following the incident, all passengers safely disembarked and were transported with the rear locomotive and the remaining 8 coaches to Abuja, where they arrived at 12:32 PM, slightly behind the scheduled arrival time of 10:17 AM. 

    “The front locomotive, the derailed BVA, the derailed coach, and another unaffected coach remained at the site of the incident.”

    Giving outcome of preliminary investigation, Oladeji said: “Preliminary fact-finding undertaken by our Investigators revealed that the train was travelling at a speed of 90 km/h in the section leading up to Jere Station and reduced to 23 km/h on approach to the station.

    “However, immediately after the northern change point at Jere Station, the BVA and a coach of the train derailed.”

    She said further probe of the incident was aimed at determining the circumstances that led to the derailment. 

    “The investigation will examine factors such as the train’s speed, track conditions, mechanical components, and operational procedures.

    “As part of the investigation process, the NSIB will collaborate with relevant stakeholders, including the Nigerian Railway Corporation (NRC), to analyse data from the On-Train Monitoring Recorder (OTMR) and other critical sources of information.

    “The safety of passengers and crew is paramount, and NSIB is committed to ensuring that all necessary measures are taken to prevent reoccurrence and improve the safety of our railway systems,.

     “Our priority is to determine the derailment’s root caused and make safety recommendations to enhance the safety of railway operations in Nigeria and prevent reoccurrence.”

    The Bureau, while stating its committment to conducting a comprehensive and transparent investigation, added that it would provide updates as the investigation progresses.

    The NSIB urged the public to exercise patience and refrain from speculating on the causes of the derailment until the investigation is concluded and factual findings are presented.

    The Nigerian Safety Investigation Bureau, has been notified and has commenced investigation into a serious incident involving a Boeing 737 with Nationality and Registration Marks 5N-BYQ belonging to Aerocontractors Airlines.

    This is contained in a statement signed by Dr James Odaudu, the NSIB Director of Public Affairs and Consumer Protection, on Sunday in Abuja.

    Odaudu said there was no injury or fatality.

    “The incident occurred around 10:47am (Local Time) on 12th November, 2023.

    “The aircraft upon landing at the Nnamdi Azikiwe International Airport, Abuja from Lagos and in a bid to exit runway via A4 had its nose wheel stuck in the grass verge with its fuselage on runway.

    “Consequently, the runway was closed pending the towing of the aircraft from the runway. There was no injury or fatality,” it said.

    According to him,the NSIB, hereby, solicits information from the general public in form of pictures, video or recording evidences to assist in conducting a comprehensive investigation.

    ”The NSIB will appreciate that the general public and press respect the privacy of the people on board and not assume the cause of the serious incident until formal report is released.

    “The NSIB is a multimodal investigation agency charged with the mandate to investigate transportation accidents and serious incidents in Nigeria with the aim of identifying the probable causes and proffer safety recommendations that can prevent reoccurrence,” it said.

    The director affirmed that the bureau would release the preliminary report as soon as possible.

    The Nigerian Safety Investigation Bureau (NSIB) said it has commenced investigation into a serious incident involving a Boeing 737 with registration number 5N-BYQ operated by Aero contractors Airlines.

    The incident occurred around 10:47am on Sunday after the aircraft landed at the Nnamdi Azikiwe International Airport, Abuja from Lagos.

    The Bureau in a statement by its Director of Public Affairs and Consumer Protection, Dr James Odaudu stated that the aircraft had its nose wheel stuck in the grass verge with its fuselage on the runway while the aircraft was exiting the runway via A4.

    The bureau said the incident led to the temporary closure of the runway.

    The statement reads: “The Nigerian Safety Investigation Bureau, has been notified and has commenced investigation into a serious incident involving a Boeing 737 with Nationality and Registration Marks 5N-BYQ belonging to Aero contractors Airlines, which occurred around 10:47am (Local Time) on 12th November, 2023.

    “The aircraft upon landing at the Nnamdi Azikiwe International Airport, Abuja from Lagos and in a bid to exit the runway via A4 had its nose wheel stuck in the grass verge with its fuselage on the runway.

    “Consequently, the runway was closed pending the towing of the aircraft from the runway.

    NSIB probes aircraft crash at Ibadan airport

    The Nigerian Safety Investigation Bureau (NSIB) said it has begun investigation into the cause of the crash of an aircraft operated by
    Flint Aero.

    The crash was said to have occurred at the Samuel Ladoke Akintola Airport, Ibadan on Friday night.

    The aircraft with type HS25B departed Nnamdi Azikiwe International Airport, Abuja on Friday with seven passengers and three crew members on board.

    The aircraft was airborne Abuja at 1841hrs and crashed into the bush short of the runway at about 1921hrs in Ibadan.

    According to an official investigating the crash, no fatalities were recorded but the aircraft sustained substantial damage.

    He said the pilot was cleared for RNAV approach Runway 22, adding that all passengers and crew were safely evacuated.

    Reacting to the crash, the Director of Public Affairs and Consumer Protection of NSIB, Dr. James Odaudu stated: “On the accident involving an aircraft operated by Flint Aero which occurred at the Samuel Ladoke Akintola Airport Ibadan, please be informed that the Nigerian Safety Investigation Bureau (NSIB) has commenced investigation into the cause.

    “Our investigators are on site, and further information will be provided as soon as possible”.

    The Nigerian Safety Investigation Bureau (NSIB), has commenced investigation into the incident involving an aircraft in the fleet of United Nigeria Airlines.

    The bureau equally unveiled the identity of the aircraft as an Embraer ERJ145 with Nationality and Registration Marks: 5N-BWY.

    The NSIB in a statement said: “The Bureau has been notified and commenced investigation into a serious incident involving an Embraer ERJ145 with Nationality and Registration Marks: 5N-BWY belonging to United Nigeria Airlines, which occurred around 06:33pm (Local Time) on 8th September, 2023.

    “The aircraft with 51 passengers and 4 crew members onboard was en-route Lagos from Sam Mbakwe International Cargo Airport, Owerri, Imo State.

    “The aircraft on landing at the Murtala Muhammed Airport, Ikeja, Lagos, skidded off the runway. There was no injury or fatality.

    “The NSIB, hereby, solicits information from the general public in the form of pictures, video or recording evidence to assist in conducting a comprehensive investigation.

     The bureau, however, urged the public and the media to respect the privacy of the people involved “and NOT assume the cause of the serious incident until a formal report is released.”

    The bureau promised to release the preliminary report soon.

    Meanwhile,   United Nigeria Airlines admitted on Saturday that flight U5 0513 operated by its aircraft with registration 5N-BWY upon landing at Murtala Mohammed Airport, Lagos, aquaplaned due to a downpour.

    Spokesperson of the airline, Achilleus-Chud Uchegbu said the aircraft which  was coming in from the Sam Mbakwe International Cargo Airport Owerri  landed normally but skidded off the runway.

    He said:” All passengers have also been safely evacuated alongside with their luggage. No passenger was hurt in the incident.

    “Relevant authorities have been informed about the incident.

    “Passenger safety is of utmost priority to  United Nigeria Airlines. We assure our passengers that we value their safety and will not compromise on it. We shall continually work, like we have always done, to ensure that our passengers are always united with their destinations safely.”

    Meanwhile, the Nigeria Civil Authority (NCAA) has stated that there are no plans to replace the runway lights of  the Mallam Aminu Kano International Airport (MAKIA), Kano with the missing airfield lights of Runway 18R of  Murtala Muhammed  International Airport  (MMIA) , Lagos.

    Its Director General,  Captain Musa Nuhu said reports suggesting that the Minister of Aviation and Aerospace Development, Olorogun Festus Keyamo, ordered such removal was false.

    Nuhu said there was no way the minister could have given such an order considering that MAKIA , Kano,  is among the five international airports in the country close to receiving certification as required by the International Civil Aviation Organization (ICAO).

    The NCAA boss clarified :” The authority wishes to assure the traveling public that it will leave no stone unturned in ensuring continuous safety of flight operations in all sectors of civil aviation in Nigeria.

    ” The  Nigeria Civil Authority (NCAA) has been notified of a report that has gone viral in the social media claiming the Minister of Aviation and Aerospace Development Mr.Festus Keyamo has ordered the removal of airfield lights at Mallam Aminu Kano International to replace the missing airfield lights at Runway 18R at Murtala Muhammad International Airport Lagos.

    “The regulatory body of civil aviation in Nigeria, the Nigeria Civil Aviation Authority (NCAA) wishes to inform the public to disregard the report, as it is not aware of any directive to remove any of the several airfield light systems at Mallam Aminu Kano International Airport.”

    NSIB, NAMA sign pact on air safety

    The safety of the country’s airspace received a boost yesterday when  the Nigerian Safety Investigation Bureau (NSIB) and the Nigerian Airspace  Management Agency (NAMA) signed a Memorandum of Understanding (MoU) to promote air safety.

    The objective of the agreement, according to a statement by the agencies, is to consolidate the  relationship between them on the exchange of information, and data sharing  during occurrences, among others.

    The agencies said the pact would help in the quality and timeliness of investigation of air mishaps in accordance with the International Civil Aviation Organisation (ICAO) Annex 13 and 19 of  the 1994 Chicago convention on International Civil Aviation.

    Also included in the MoU,  is the flexibility in all transactions with the mutual agreement of both agencies, upholding the values of cooperation, honesty, mutual respect, openness and professionalism.

    NSIB and NAMA  noted  that though the MoU  is not legally  binding, the two agencies agreed to  perform their functions in consonance with the principles  of the deal.

    The event, which took place at the NSIB Abuja headquarters. The Bureau’s Director-General, Akin Olateru, signed on behalf of NSIB, while NAMA Managing Director/Chief Executive Officer, Odunowo Tayib Mohammed, for his agency.

    The NSIB recently signed a MoU with Lagos State Emergency Management Agency (LASEMA) on cooperation during emergencies.

     even as the agency has signed similar agreements with various agencies and institutions within and outside the country.

    Said  Olateru, “Collaboration remains the centre point of our drive towards enthroning safety in the aviation industry. We remain committed to promoting safety in the aviation industry to the point of attaining and maintaining zero accident.”

    He added: “We, however, cannot do it alone. That is why we have chosen the path of collaboration with agencies and institutions with similar objectives.”

    He commended NAMA management for rising to the occasion  by signing the agreement.
