  • ‘Nigerians need therapy to overcome their daily stress’

    Founder of Therapists Without Borders Foundation, Dr. Tosin O. Emmanuel, a PhD. holder in Occupational therapy, speaks with Dorcas Egede on the profession, its relevance in a society like Nigeria and the humanitarian efforts of her organisation.

    MORE people are familiar with physical therapy than they are with occupational therapy. What’s the difference?

    Physical therapy and occupational therapy are parts of rehabilitation medicine. However, they differ in scope of practice. Although there are some grey areas where these practices collide with the services they provide, with regards to functional mobility and ambulation. Occupational therapy differs from physical therapy in that occupational therapy provides holistic services whereas physical therapy is more biomechanical. Occupational therapists evaluate the overall aspect of the individual, beginning with his medical diagnosis, then his functional deficits, followed by his areas of occupation and lastly his reintegration into his community. In other words, we assess the overall well-being of the patient to ensure that they are able to engage in activities that are productive and meaningful to them. Physical therapy on the other hand, focuses more on the anatomical deficits patients convey during their illness.

    What exactly do you do as an occupational therapist?

    My areas of practice focus on improving patients’ function to safely and inde

    pendently perform activities of daily living, with aims to improve their overall quality of life. These interventions include providing intricate therapeutic services to patients with conditions that are mentally, physically, developmentally, socially, and emotionally disabling.  Some of the services I provide include ADL and IADL retraining,

    activity modification, fine motor and gross motor retraining, cognitive skills retraining, cognitive-behavioral therapy, visual-perceptual skills retraining, sensory-processing interventions, Lee Silverman Voice Treatment (LSVT) Big techniques to Parkinson patients, integrated client-centered spinal-cord programme, and the overall activities that promote community reintegration.

    People generally think of therapy as being for sick people. What is your take on this?

    Therapy is not just an intervention for people who have physical and mental disorders. Therapy is for everyone. Actually, therapy is a form of intervention to help an individual feel better and become stronger after a stressful event. Therapy is usually stigmatised with a negative connotation that usually makes people reject and condemn the services in general. However, I believe that through constant education and awareness within our communities this negative stigma will decrease. Therapy in general helps improve function and enhance our overall quality of life.

    What are the benefits of therapy in Rehabilitation Medicine?

    Therapy is beneficial to everyone but most especially those experiencing physical and psychological deficits. Therapy helps to reduce pain or even eliminate pain, improve mobility, manage diabetes and vascular condition, by providing a diabetes management plan which would include exercise, foot care education, and proper dieting education. Therapy helps to manage

    age-related issues by teaching patients how to manage arthritic or osteoporotic conditions conservatively. Therapy assists in managing heart and lung disease by educating patients on proper breathing exercises. Therapy helps to manage women’s health and other conditions. Lastly, therapy helps to restore function, both physically and mentally, and improve patients’ overall quality of life.

    Can you explain the term occupational hazard, vis-à-vis developed countries and Nigeria. Does it really apply here?

    Occupational hazard is a universal term that signifies both long-term and short-term risks associated with the workplace environment. So, be it in Nigeria or in the Diaspora, as long as people work, they are prone to occupational hazards in the long run. These hazards, depending on the profession, include low back pain and musculoskeletal problems, which can be a result of poor sitting posture at a computer desk for prolonged periods of time or the overall poor body mechanics when lifting heavy objects. In this case, occupational therapists will provide workplace ergonomic training to the employer and the employees to adapt or modify the work place environment or task, and help facilitate successful return to work after an illness or injury.

    Occupational therapy isn’t such a common profession in Nigeria; how popular is it in the developed world?

    Occupational therapy is a common profession in the Diaspora. It is a branch of Rehabilitation Medicine, which also includes physical therapy and speech pathology. Occupational therapy is the link between physical therapy and speech pathology. However in Nigeria, the lack of awareness about the occupational therapy profession is perplexing. As a doctor of Occupational therapy based in the United States and slowly migrating to Nigeria, I believe the first step is to have the programmes in every university in Nigeria, beginning with the Bachelor’s Degree and slowly building up other degree programmes, as they have in India and other western countries.

    Tell us about the Therapist Without Borders foundation.

    Therapists Without Borders Foundation (TWBF) is a non-profit organisation I founded in 2015. It supports the provision of therapeutic services, apists analyse an activity in detail to determine the necessary components to perform the task, which is a unique core skill of occupational therapists, based on their education and training.

    What do you advise when people are stressed at the workplace?

    My therapeutic advice is to first understand your stressors by writing them down in a book or journal. Then you develop a positive way to address each stressor. Establish boundaries or control the environment by stepping away for some minutes to recuperate. LeaArn your mode of relaxation, such as deep breathing exercises, listening to music or providing yourself with positive words. Lastly, talk to your supervisor- effective communication in a work place is vital.

    Can you share a situation during your practice where you felt most proud to be an Occupational Therapy practitioner?

    One major success that still resonates within me was when I had an opportunity to treat a patient who had a C5 complete spinal cord injury from a motor vehicle accident. She was transferred to our facility from a well renowned hospital, Helen Hayes, in New York. Helen Hayes is known for their exceptional rehabilitation services – in fact, they treated the famous actor Christopher Reeve, known as “Super Man,” after his spinal cord injury.

    When my patient arrived to my facility, she was bed-ridden with a head brace and an abdominal binder. She had no movement in her upper extremities and lower extremities due to the accident. I must say after a year and 6 months with an aggressive occupational therapy that lasted about 2.5 hours daily for 5 days in a week, my patient was able to move her bilateral upper extremities. In addition to this, she exceeded all the research expectations and began utilising a universal cuff brace to engage in self feeding and grooming tasks with set-up. She also was able to utilise the universal cuff brace to operate her motorised wheelchair, in order to have access to independently move around the facility. She is my success story. She is now able to independently operate her iPhone to make calls, send text messages and emails while using the universal cuff brace. She is able to safely and independently operate the elevator within our facility with her motorised wheelchair. To top it all, she is back in her community with her 24 hour aide and reunited with her children, which was her major goal upon her admission into our facility. Consequently, I was honored to be the 2018 National Nursing Home’s week Face of Rehabilitation in my company.

    In what ways can one go through therapy in a stressful city like Lagos, without visiting the hospital?

    Lagos is a stress city and there are some events we can’t control on a daily basis. These events such as traffic, provision of electrical power and water supply and the psychological state of people we meet daily are some examples. However, we can try to make it less stressful by incorporating various therapeutic strategies such as

    1. Engaging in deep breathing exercises during a stressful event helps serve as a form of relaxation technique.
    2. Engaging and encouraging positive thoughts helps to provide some form of calmness to our mind
    3. Engaging in visual imagery helps to relax the tensions in our muscles.
    4. Engaging in music therapy uplifts mood during a stressful day

  • ‘Occupational therapy lucrative profession’

    Potential students seeking admission have been urged to embrace the Federal School of Occupational Therapy, Oshodi.

    According to the Medical Director of Federal Neuropsychiatric Hospital, Yaba, Lagos, owners of the school, Dr Oluyemi Ogun, the field of occupational therapy is lucrative.

    She stated this at the institution’s matriculation of 58 intakes.

    On the relevance of occupational therapists in healthcare, Mrs Ogun said they help to improve patients’ability to perform  tasks at work, home, and leisure, among others and facilitate their successful adaptations to disruptions in lifestyle, prevent losses of function and improve or maintain psychological status.

    The institution’s Registrar, Olori Lydia Ajayi, said the school needs  more funding. She explained that the formation of the school is to fill the shortage of occupational therapists.

    She explained: “The idea of establishing a School of Occupational Therapy was conceived by the former Medical Director of the hospital, Dr Idowu Malomo who did not only have the foresight to recognise the apparent dearth of this noble profession in Nigeria but also went ahead to ensure that something was done about it. Before the school commenced, the Management of Federal Neuro-Psychiatric Hospital had been desirous of meeting up with international standards in the provision of care for the mentally ill but the hospital was incapacitated in the area of having well-trained staff who could take up this need. There was scarcity of professionally qualified occupational therapists in the country.”

    She added:  “The Management Board for the hospital then set up a Technical Committee under the leadership of Dr. Obal Otu. This committee put in their best and came up with reports and several working papers all set towards the actualisation of this goal. The Management Board, based on the Technical Committee reports, sought the approval of the Federal Ministry of Health and this was given. The minister also granted the use of the abandoned property near the hospital for the take-off of the school immediately. The Federal School of Occupational Therapy was approved for establishment by the Federal Ministry of Health on   March 15, 2002. A prominent member of the Technical Committee that facilitated the approval was the late Mr. J.T. Adamu, Deputy Director (Nursing).  He, sadly, did not live to see the school take off in March 2003.”

    Olori Ajayi boasted that the institution is the first of its kind in Nigeria and the West African Sub-region.

    The Acting Principal of the institution, Mr Adeoso Akinwole, said: ‘’We have produced best of hands that are manning various hospitals in both public and private practice.”

    Acting Medical Director of the Federal Neuro-Psychiatric Hospital in Yaba, Lagos Mainland, Dr Richard Adebayo has urged fresh students of Federal School of Occupational Therapy in Oshodi to acquire skills that will make them experts in their field.

    Speaking at the students matriculation, he reminded them that the institution could only award diploma, noting that they could study for their first degrees after the diploma course.

    He said: “We have an understanding partnership with the Obafemi Awolowo University (OAU) in Ile-Ife for admission of our students into 200-Level for Bachelor’s degree in Medical Rehabilitation. There are also other universities abroad that accept the certificate for various degree programmes. I want to challenge you to work hard and get the best results possible so that your admission into universities would be easier.”

    According to Adebayo, the hospital set up the Occupational Therapy programme to train manpower for the field. He said the hospital employed indigenous professionals and expatriates to train the students.

    He said: “The Psychiatric Hospital set up this school 14 years ago to increase the number of professionals in Occupational Therapy, which is highly relevant in the therapeutic treatment of people suffering mental illness as well as physical disabilities. Professionals in this field help mentally-challenged individuals to develop skills needed to live independently. It is the only profession that helps people to do things they want and need to do through the use of therapeutics.”

    The Acting Principal, Mr Adeoso Akinwole, said students were trained with world-class facilities that would make them compete with their peers worldwide. He said trainees could work in hospitals, rehabilitation centres, nursing facilities, outpatient clinics, schools and private firms.

    The Registrar, Mrs Lydia Ajayi, said the hospital was exploring ways to ensure better learning environment for the students. “I congratulate the 39 of you that are matriculating today. Management has awarded contract for the construction of a multipurpose skills acquisition centre that will comprise a wood workshop, a laboratory, library,  information technology studio and other facilities. This is to ensure that students are exposed to activities relevant to their training,” she said.

    Adult Stem Cell Therapy is new in Nigeria. David Ikudayisi of Glory Wellness and Regenerative Centre, Lagos, says it has many benefits. In this interview with OYEYEMI GBENGA-MUSTAPHA, the medic urges Nigerians suffering from non-communicable diseases and infertility to go for the therapy.

    It is believed that Stem Cell Therapy involves the use of embryos, hence, peoples’ reluctance to accept it. Is this true?

    There are two main types of stem cells – embryonic stem (ES) and adult stem cells. The former involves the use of embryos as source of cells while the latter involves the use of cells harvested from an individual or adult. Others, such as induced pluripotent stem cells (iPSCs), are produced in the laboratory by reprogramming adult cells to express embryonic stem cells characteristics. The type of stem cell therapy that we are talking about here is the adult stem cell therapy, and this uses a patient’s own body fat or bone marrow as the source of stem cells, unlike the controversial embryonic stem cell therapy. Regenerative medicine aims to restore normal function by repairing or replacing damaged or malfunctioning cells and tissues in patients who have lost tissue or organ function due to age, disease or congenital defects. With PRP therapy, the growth factors in the blood activate the healing process at a faster rate. And with adult stem cell therapy, the stem cells in adult fat/bone marrow can differentiate into any tissue where they find themselves. Adult stem cell therapy is new in the country but the Nigerian doctors are aware of the use of bone marrow transplant for blood disease called leukemia. In the past, PRP therapy has been used in at least one of our teaching hospitals in Lagos; some doctors are now using the therapy. PRP therapy is very popular among orthopaedic doctors in Nigeria, but there are still drawbacks due to the cost and logistics for the state- of-the-art technology, and affordability by patients. Meanwhile, adult stem cell therapy is new in Nigeria, but not in Europe, United States or India.

    You mean Stem Cell Therapy can  treat many ailments?

    The benefits of adult stem cell therapy are enormous, and there is much more to be discovered. The basic way to understand who will benefit is simply knowing that if there is need to repair and/regenerate any part of the human tissue/organ, then adult stem cell therapy with/without PRP therapy may be an option. An exception is the treatment of cancer. Adult stem cell therapy can be used for shrink prostate and brain cancer the same day the stem cells are harvested from the patient, but not for the rest of the cancers without first doing tissue engineering of the stem cells in the research laboratories. Another exception is sickle cell anaemia treatment, which requires stem cells transplant from another person unlike adult stem cell therapy because in adult stem cell therapy, the stem cells are taken from the same patient that will be the recipient of the stem cells and all this is done the same and in the presence of the patient.

    For some of the common treatable ailments, they are in two categories: Local and systemic applications. Under applications, you have ailments, such as multiple joint pain, back pain, meniscal tears, ligament tears, avascular necrosis of the hip joints, facet arthropathy, plantar fasciitis, post-surgical hip pain, chronic non-healing wounds, dental procedures, hair thinning, erectile dysfunction, female sexual dysfunction, female urinary incontinence and cosmetic/aesthetic applications (vampire facial, vampire facelift, vampire breast and nipple lift.

    The ailments treatable under systemic applications  include diabetes, hypertension, aging (generalised treatment), fatigue, fibromyalgia, chronic kidney disease, lupus, rheumatoid arthritis, scleroderma, multiple sclerosis, Parkinson’s disease, congestive heart failure, heart attack (myocardial infarction), stroke, brain injury, Alzheimer’s disease, autism, spinal cord injury, leukemia, COPD (lung disease), Crohn’s disease, amyotrophic lateral sclerosis (ALS), and infertility, etc. As for the infertility treatment, it increases the chances of becoming fertile and/or the success rate of Invitro Fertilisation (IVF).

    But the Medical and Dental Council of Nigeria (MDCN) has its regulation on Stem Cell Therapy.

    MDCN regulates all practices of medicine in Nigeria. Although adult stem cell therapy may be new, PRP therapy is not new as it is common among doctors in Nigeria. Embryonic stem cell is not being practiced in Nigeria to the best of my knowledge, and that is not what we are currently talking about anyway. Also, I am not aware of any research going on or being conducted by the Nigerian government or a hospital about induced pluripotent stem cells (iPSCs) therapy.

     Are there side effects in this therapy?

    No.The stem cell treatment we are talking about is autologous, i.e. taken from the same patient that will be the recipient. Therefore, there is no risk of rejection of the transplant.  Let me also say that the PRP in Platelet Rich Plasma (PRP) therapy has antimicrobial properties.

    If the therapy has so many benefits, why are people not partronising it?

    Adult cell therapy is relatively new in Nigeria. Unfortunately, and unlike in the USA, most Nigerians do not believe in preventive medicine. They only go to doctors when it is almost too late or at the precarious stage of their conditions. I do not blame them because the main reason for delaying health care is the financial cost. I hope that, one day, we will get to that level of health care system that is available in developed countries. We are just trying to create awareness on regenerative medicine in Nigeria. Nigerians keep travelling to India to get some of the procedures done, including the ones involving adult stem cell therapy. It will be great if many of the Nigerian doctors in USA, U.K. and other advanced countries, who practise regenerative medicine, will consider bringing their practice and knowledge home for the larger population of Nigerians to also benefit. I am not asking them to do pro bono because I understand the cost implication of my request, but, at least, let us start from somewhere. Nigeria, too, can become a centre for medical tourism in Africa because of regenerative medicine. There is a future for Nigeria in the medical world. For example, open heart surgery was performed in 2015 at Babcock University, and there are some world- class Intensive Care Units in Nigeria that I am aware of.

    What edge does stem cell therapy has?

    Stem cell therapy is not for everyone. It can reverse the symptoms of some diseases; it aims at regenerating and/or repairing the damaged tissue in the body which conventional treatment does not do. There is limited life span for surgical and other procedures; there is a limitation for surgery for spinal problems. For example, not everyone with bad knee joint wants to go for knee surgery, if it is treatable by another means, especially if the patient prefers to use his  body to heal himself. Conventional treatment does not last as long as the effect of adult stem cell therapy or PRP therapy in conditions/ailments where regenerative medicine is applicable and effective. Intravenous application of Adult Adipose (Fat) Stem Cell Therapy is also good for Anti-Aging. Not everyone wants to take pain medication on daily basis if that can be avoided. There are situations where conventional medicine has no suitable answers. For example, no one time or single dose medication for hair thinning, no medication can regenerate kidney cells to prevent them from deteriorating, no medication for female sexual dysfunction affecting married women, no medication reverses the symptoms in patients with severe multiple sclerosis, just to mention a few.

    Where can this therapy be procured?

  • Tips for leukaemia, sickle cell disorder therapy

    When Seun Babalola called me about three weeks ago and asked for first aid therapy for a middle-age sickle cell man in deep crisis, I scribbled a note which formed the basis of today’s column.

    As a note, it cannot be comprehensive. But it should be useful for people who wish to avert a crisis or manage one.

    As I was scribbling the note, a young Mass Communication graduate of Moshood Abiola Polytechnic, Abeokuta, Miss Morayo Fadele, who was in my office, asked if I could broaden the tips to include a therapy for leukaemia (blood cancer).  Her father bowed to this condition a few years ago. And, as she said, she would like to be of help to anyone challenged by it. I apologised that home or clinic therapies for leukaemia would have to wait. But I made a few suggestions which included stinging Nettle, Yarrow, Blood root, Horsetail, wheatgrass, Chlorella etc.

    Leukemia is cancer of the blood. And the blood is the river of life flowing in our blood vessels. The blood has a specific composition. Anything outside this natural formula is at variance with healthy blood and can spell doom. This, constant recomposition of the blood to maintain the natural formula is crucial for averting blood disaster. But before we can recompose, we must clean out foreign elements from the bloodstream. This will lead us to detoxification.  All blood filters in the body must be at work and efficient. The liver, the kidneys, the lungs, the skin and the digestive tract are important for this purpose.     • The liver has a duty to conjugate this heavy bilirubin load which can become poisonous to the system, especially the brain, if not promptly transform (conjugated) into less harmful substances. It is this heavy congestion of bilirubin in the liver that makes the liver becomes inflamed and painful. The spleens, too, may become inflamed and painful, soft and purple, it is vascular and lies in the upper left region of the abdomen. As a filter for the blood, it removes all old or malformed or damaged red blood cells.

When the mineral selenium is taken with vitamin E, vitamin E performs better. The better it is if  grape seed extract is added to the list, being about 50 times more powerful than Vitamin E. When you purchase vitamin E, be sure to buy the natural (d-alpha) and not the cheaper dl-alpha which come from petroleum sources. Be sure, too that the d-alpha contains the full components of d-alphaTocopherols.

    Shark liver oil: One of the important components of this oil is SQULENE. It helps the shark to survive in the depths where oxygen is low.     • The Bone Marrow: Bone pain can be eased with the forgoing suggestions. Comfrey Root Tea improves bone health and curbs the pain. So does Yarrow tea which has an affinity for the bone marrow. Beet Root is also good. Comfrey root powder can be mixed with shea butter and used as a massage cream on the painful bone.

    Diatom, also called Diatomaceous Health, is good too, especially as it is an alkaliniser and it has an affinity for the bone. As it strengthens connective tissue throughout the body, strengthening connective tissue in the bone marrow prevents the tearing which inflammation induces and of course, the pain which comes with tearing. The weapon in the hands of diatom is SILICA.

    Silica is present also in Horsetail and stinging Nettle. But it accounts for between 92 and 96 percent of Diatom. Recently I asked a throat cancer patient to try it. She had a terribly ulcerated stomach, and fed through a tube in her stomach. Whenever she passed liquefied food through this tube, her stomach was literally on fire, but when she ground two tablet of zinc to powder and passed it down with water, the pain eased as the zinc supported the healing process. And any time she felt the pain was about to come up and she fed Diatom to the stomach, the pain disappeared outright.

     

    • CLUMPING: Gumming (clumping) of cells or agglutination is one of the problems faced by a sickle cell challenged person. Clumping slows blood circulation, and, thus, delays oxygen and nutrient delivery to the cells as well as waste or poison evacuation from them. During clumping episodes, the cells are, thus, starved and malnourished and left to live in a mesh of poison. Natural blood thinners’ free them from this peril. Some naturals include Onion and Garlic. They should be present always in the diet.

    Kyolic garlic, a proprietary brand of garlic, is reported to be about 50 times more potent than natural garlic, having been specially aged for about 15 months. The proprietary product Curcumin 2000X is good for this condition.

    Curcumin is anti-inflamating and helps all the inflammation caused by this challenge. The Cayenne present in Curcumin 2000X is reported to make it 2000 times more active than when curcumin is taken alone. Cayenne is a blood thinner and drives the blood to every nook and cranny of the body. Cayenne may be used on its own where curcumin 2000X is not readily available. I add cayenne to my corn portage (pap) and herbal drinks. Serrapeptase is another good blood thinner which solves blockages in the system. In the cell or tissue salt category of food supplement, Ferrum phosphate disbands all congestions, oxygenates the blood.
